Make a Referral

Who can be referred?

To be eligible, the service user will be:  

   ·         aged 11 to 18
   ·         with severe physical disability and / or health difficulties
   ·         have appropriate funding for short breaks in place

How to refer

Referrals can be made by: 

   ·         Telephone
   ·         Email
   ·         Using the website online referral form

What happens once the referral is made?

   ·         We contact you and discuss your requirements
   ·         We will advise you of our fees

If you wish to proceed:

   ·         We will undertake an initial assessment of the young person and their needs
   ·         We will make a formal booking
   ·         Work through the our 'breaks process' with Social Worker / Carer and Young Person
